Please call to discuss current fees for the various services. Although, I do not bill insurance directly, I will provide you with a billing statement with the information required for you to submit to your insurance.
Payments can be made at the time of service or at the end of each month. Cash, check, and payment through Venmo are accepted. You can learn more about Venmo here.
I can be contacted by phone, text, or email. Please note that email is not considered secure and medical confidentiality can be compromised. During the week, I will respond to you within a 24 hour period. During the weekend, I check messages and will get back to you as I am able.
While on vacation, I will have a colleague on call for me, and if needed I will arrange someone to see you in my absence.
If at any time you have an emergency, and cannot wait for my return call, go to your local hospital emergency room or call the Crisis Hotline at (408) 279-3312.
